Understanding the Program

To access our home care services, individuals must undergo an assessment by Home and Community Care Support Services (HCCSS). This evaluation helps determine eligibility and creates a personalized care plan tailored to specific needs.

Who Qualifies for Our Services?

• Children with complex medical needs.
• Adults with acquired brain injuries.
• Eligible homeschooled children.
• Patients in extraordinary circumstances, as evaluated by HCCSS.

What Are Extraordinary Circumstances?

• Require intensive care.
• Face unique scheduling issues.
• Have language or communication barriers.
• Need culturally specific care.
• A person is located in areas lacking HCCSS contracted service providers or in remote locations.

Your Role in the Program

As a substitute decision-maker, you will manage and coordinate your loved one’s care. This includes hiring and overseeing caregivers, handling the financial aspects of care, and ensuring regular communication and reporting to Home and Community Care Support Services (HCCSS).
